The team at KuniDesign.com added a custom form almost immediately to the website that they built in concert with Ubertor for the people of the Philippines. On the homepage of www.helpondoy.com you will see a custom form at the bottom that is specifically built for the needs of that website.
And the designers at SplitMango.com wrote a blog post about the new Ubertor feature with an example of how it can be used within a widget on any page within an Ubertor website.
